There's a WordPress function for that which allows you to provide the parent of the new term: http://codex.wordpress.org/Function_Reference/wp_insert_term

There are also many functions available to get and manipulate terms of posts, like http://codex.wordpress.org/Function_Reference/wp_get_object_terms etc.
