Double input(input related to another input)

I tried to work this out to achieve the form custom field input to have another input which sets the condtion of the side by side input,
For example,

I would like to input HUMAN, then its condition whether his Passport is ready or not, just a simple OK or not ok.

I understand to achieve this I have to create a custom field of passport ok, and another passport not ok, and add the HUMAN names to it, which is very hassle if the status of passport changed and i have to delete from passport not ok and add it again into passport ok.

Please anyone can help me sort it my way?
