Can't access CRED form

I created a Custom Post Type using WP-Types. I then created a CRED form to Create and Edit the custom post type. I used Access to specify that "Contributor" has permission to view/edit/publish the custom post type and access the CRED front end form.

I can access the CRED form when logged in as an administrator, but when logged in as a Contributor, it won't show the CRED form.

What am I doing wrong?
